Job Description:
As a Behavioral Health RN Case Manager, you will have the opportunity to impact lives and be part of something greater.
Key Responsibilities:
- Take ownership of a caseload of behavioral health patients, assessing their overall health and psychosocial needs, and ensuring they receive the care they need.
- Collaborate with the interdisciplinary team to create personalized care plans, adjusting as necessary to address changing patient needs.
- Work closely with physicians, clinical supervisors, and healthcare team members to ensure timely interventions and excellent patient care.
- Provide valuable education and support to patients, families, and facility staff, ensuring the best possible care at all times.
- Supervise and guide behavioral health aides and LPNs, ensuring consistent, high-quality care.
Requirements:
- Current RN License (unrestricted in the state)
- 1+ year of clinical experience (Behavioral Health, Home Health, or Hospice preferred)
- Reliable Transportation: A dependable vehicle, valid driver's license, and auto insurance are required, as you will travel within the coverage area.
- 50% Travel Required: Be ready and willing to visit patients across the service region.
