Part-Time Nurse Practitioner for In-Home Health Evaluations

As a Nurse Practitioner with Signify Health, you will conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. This flexible nursing role allows you to make a meaningful impact on people's health and well-being while enjoying a competitive wage and flexible schedule.
- Conduct in-home visits focusing on a person's overall health and well-being
- Earn up to $2,500 to $4,000 each week (Average 6-9 visits daily, base rate of $80 per completed assessment)
- Increase your per-visit rate up to 135% with our Market Multiplier
- Shifts available Monday - Sunday (no nights/weekends required)
- Short-Term or Long-Term basis
- Pre-scheduled visit in-home w/ health plan members
- Airfare, Hotel, and Rental Car available (depending on eligibility)
- Malpractice coverage
- Availability Incentives (when you offer availability to complete five appointments per day worked)
- Independent Contractor, 1099 status
- Flexible Schedule
- State License Required
These visits include a medication and medical history review and a physical evaluation performed in a health plan member's home. Our NP contractors can earn a competitive wage (or supplement their current income) while managing other personal and clinical commitments.
