Clinical Liaison

We are seeking a highly skilled Clinical Liaison to join our team at Community Medical Center. As a Clinical Liaison, you will play a critical role in identifying patients for potential admission into our inpatient acute rehabilitation program.
Key Responsibilities:
- Interact with patients and families to gather clinical information and past history
- Conduct thorough patient assessments to identify patients for potential admission
- Educate physicians on quality, programs, accreditations, and regulations related to inpatient acute rehabilitation
- Develop educational and community relations plans in consultation with Program Director, Medical Director, and Nurse Manager
- Provide in-service/cross-training to team members on acute rehabilitation criteria, diagnostic case mix management, coding, etc.
Requirements:
- At least a 2-year degree in a health-related field, with a bachelor's degree preferred
- Previous marketing/sales experience preferred
- Applicable and current license to practice in accordance with state licensure act
- Valid driver's license and reliable transportation required
- Strong interpersonal skills and experience working with physicians, nursing management, discharge planners, case managers, and social workers
