Job Summary

Sapphire at Rivers Edge is seeking a dedicated Licensed Practical Nurse (LPN) to join our team in providing exceptional care to our residents. As a LPN, you will be responsible for delivering high-quality, person-centered care, coordinating with interdisciplinary teams, and ensuring compliance with all regulatory standards.

Responsibilities
  • Provide primary nursing care to residents with varying backgrounds and medical histories
  • Assist with new admissions and monitor resident needs
  • Communicate changes of condition to providers and implement medical interventions as needed
  • Communicate with outside providers, including primary care providers, home health agencies, and pharmacies
  • Implement and follow primary care providers' orders
Requirements
  • Licensed Practical Nurse (LPN) licensure in the state of practice
