Shelter Placement Specialist

PathForward is seeking a Shelter Placement Specialist to join our team. The successful candidate will work closely with residents to secure safe alternative housing placements or permanent housing, as quickly as possible.
About the RoleThe Shelter Placement Specialist's primary focus is to end homeless housing crises by rapidly returning residents to safe alternative housing. Key responsibilities include:
- Completing an initial intake within 72 hours of the client entering the program and following up with a comprehensive assessment after 7 days using the VI-SPADT in accordance with the CAS System in HMIS.
- Developing Individual Housing Plans (IHPs) within 10 days, focusing on identifying client needs related to housing placement, strengths, challenges, and available resources.
- Initiating housing-focused case management services for all clients within 7 days, assessing individual clients with high barriers at 30 days, and requesting a case conference when necessary.
- Incorporating post-placement services and contact follow-up after housing, developing a housing stabilization plan prior to exiting shelter.
To be successful in this role, you will need:
- A BSW or Bachelor's degree.
- Experience working with substance abuse, mental health issues, and chronically homeless individuals.
- Experience with Motivational Interviewing and progressive methodologies.
- Experience working in multicultural settings.
- CPR/First Aid Certification.
We offer a competitive salary range of $45,000 - $65,000 per year, depending on experience, plus benefits including medical, dental, and vision insurance, paid time off, and opportunities for professional growth and development.
