About the Role

This is a full-time position responsible for coordinating patient care activities, including scheduling appointments, communicating with healthcare providers, and maintaining accurate records.

Key Responsibilities

  • Coordinate patient care activities, including scheduling appointments, arranging transportation, and ensuring timely completion of necessary paperwork.
  • Communicate effectively with patients, families, and healthcare providers to ensure smooth care transitions and resolve any issues that may arise.
  • Maintain accurate and up-to-date patient records, including medical histories, treatment plans, and progress notes.
  • Assist nurse specialists with data entry, charting, and other administrative tasks as needed.

Requirements

To be successful in this role, you will need:

  • A current RN license and 1+ years of experience in home health care or a related field.
  • Excellent communication and organizational skills, with the ability to multitask and prioritize tasks efficiently.
  • A passion for delivering high-quality, patient-centered care that promotes health, wellness, and dignity.
  • Basic computer skills and familiarity with electronic medical records (EMRs).

Compensation and Benefits

The estimated salary for this position is $45,000 - $60,000 per year, depending on experience. We also offer comprehensive benefits, including health insurance, retirement plan, paid time off, and opportunities for professional growth and development.
