Street Outreach Coordinator

Primary Health Care Inc is seeking a highly motivated and compassionate Street Outreach Worker to join our team. As a key member of our outreach team, you will be responsible for conducting street outreach in the community to connect with unsheltered individuals and provide them with access to essential services.
Key Responsibilities:
- Conduct weekly outreach to non-traditional settings such as camps, congregate meal sites, and jail to ensure that unsheltered persons are connected to necessary services.
- Provide referrals to clients based on their eligibility for healthcare and/or mainstream resources.
- Answer and respond to client contact, including phone calls, in a timely manner and record accurate and complete notes in HIMS according to PHC policies and procedures.
- Assist with client transportation according to PHC transportation policy.
Requirements:
- Bachelor's degree in a related field or equivalent combination of education and experience.
- Two years of work experience in a social work setting.
- Excellent customer service skills.
- Effective written and verbal communication skills.
- Team-oriented with the ability to work collaboratively.
- Basic computer skills with proficiency using Microsoft Office applications.
- Detail-oriented with a high degree of accuracy.
- Organization and prioritization skills.
- Establishes and maintains effective working relationships with a diverse group of individuals.
- Demonstrates professionalism with the ability to maintain composure in all situations and show empathy.
Preferred Qualifications:
- Bachelor's degree in health care, psychology, counseling, mental health, human services, or a related field.
- Demonstrated competency using HIMS.
- Working knowledge of community resources related to homelessness in Des Moines.
- Knowledge of program terminology.

- A competitive hourly rate of $21.25-$25.50 per hour, depending on experience.
- A comprehensive benefits package, including generous PTO accrual, paid holidays, license/certification fee reimbursement, and more.
