Job Summary

We are seeking a skilled Licensed Vocational Nurse to join our team at Gateway Community Health Center. As a key member of our clinical staff, you will play a vital role in providing high-quality patient care and support services.

Responsibilities
  • Administer medications, immunizations, and treatments as prescribed by physicians.
  • Conduct physical assessments of patients and implement appropriate care plans.
  • Implement and monitor case management service plans to ensure optimal patient outcomes.
  • Provide education and support to patients and their families to promote health and wellness.
  • Collaborate with other healthcare professionals to ensure seamless patient care.
  • Participate in quality improvement initiatives to enhance patient safety and satisfaction.
Requirements
  • Graduate of an accredited LVN program.
  • Active license as a LVN by the Texas State Board of Registered Nurse Examiners.
  • Minimum of two years of nursing experience preferred.
  • BLS Certificate required.
  • Bilingual in English and Spanish preferred.
Working Conditions

This position requires standing, walking, stooping, bending, and kneeling. Ability to lift up to 50 pounds maximum with frequent lifting and/or carrying objects weighing up to 25 pounds.

Must be able to travel between clinic locations in Webb County, Zapata County, and Jim Hogg County.
