KEY RESPONSIBILITIES OF THE HOSPICE REGISTERED NURSE/CASE MANAGER

  • Design, organize, and deliver hospice care in alignment with the patient's individualized care plan.

PRIMARY DUTIES INCLUDE

  • Facilitating hospice care by collaborating with team members and engaging with patients and their families to address their needs.
  • Conducting assessments for hospice care and providing informational visits as required.
  • Consulting with the attending physician and Hospice Medical Director regarding any changes in the patient's condition that necessitate modifications to orders or care plans.
  • Coordinating patient services with other members of the Interdisciplinary Group (IDG) and contracted services as necessary.
  • Submitting a weekly schedule of patient visits.
  • Participating in IDG meetings and case discussions.
  • Communicating with supervisors if the IDG identifies potential benefits from contracted services for the patient.
  • Ensuring that patients' care plans are consistently updated.
  • Actively engaging in Quality Assurance and Performance Improvement (QAPI) meetings and initiatives.
  • Providing education to patients and their families about the dying process, care plans, and assisting with healthcare decisions.
  • Offering dietary advice, taking into account health concerns and dietary adjustments at the end of life.
  • Conducting death visits in accordance with agency policies.
  • Involvement in the patient's discharge planning process.
  • Maintaining current knowledge of drug therapies.
  • Complying with federal, state, and accreditation standards, including Medicare and Medicaid regulations.
  • Filling in for other nursing staff as requested by a supervisor.
  • Performing initial and ongoing comprehensive assessments of patients.
  • Gathering medical histories from patients and/or family members, particularly concerning their current conditions.
  • Explaining nursing and agency services to patients and families as part of care planning.
  • Delivering nursing services, treatments, and preventive procedures that require specialized skills as ordered by the physician.
  • Monitoring signs and symptoms, reporting any reactions to treatments, including medications, and changes in the patient's physical or emotional state to the physician.
  • Supervising and evaluating the care provided by Hospice Aides at least once every 14 days and as needed.
  • Providing guidance and oversight to Licensed Practical Nurses (LPNs) and supervising them at least every 30 days.
  • Preparing care plans for Hospice Aides.

QUALIFICATIONS FOR THE HOSPICE REGISTERED NURSE/CASE MANAGER

  • Must be a graduate of an accredited nursing program.
  • Must hold a valid Registered Nurse license in the State of Arizona.
  • Demonstrated excellent assessment and problem-solving abilities.
  • Strong organizational skills and knowledge of time management and case management processes are essential.
  • Must possess a valid driver's license and have access to a reliable, insured vehicle.
  • Effective verbal and written communication skills are required.
  • No restrictions on RN license; must be capable of administering medications, performing medication reconciliation, and executing nursing procedures or specialized treatments within the state's nursing practice scope.
