Job Overview

We are seeking a skilled Healthcare Provider to join our team in Woodstock, GA. As a key member of our healthcare team, you will play a vital role in educating patients on creating and maintaining a wellness-focused lifestyle that promotes longevity and quality of life.

Key Responsibilities
  • Conduct comprehensive patient assessments and develop personalized care plans
  • Provide education and support to patients on healthy eating habits, exercise, and lifestyle modifications
  • Collaborate with other healthcare professionals to ensure seamless patient care
  • Document patient progress and maintain accurate medical records
  • Participate in ongoing training and professional development to stay up-to-date on the latest weight management techniques and best practices
Qualifications
  • Valid Medical License
  • DEA Certification (Preferred)
  • Active CAQH profile and credentialed provider (Preferred)
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment
Work Schedule
  • Tuesday: 7 AM – 1 PM
  • Wednesday: 1 PM – 7 PM
  • Friday: 7 AM – 1 PM
  • Saturday: 7 AM – 1 PM
