Value-Based Care Coding and Wellness Visits Clinician, Advanced Practice

We are seeking a highly skilled Advanced Practice Clinician to join our team in Utah. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and education in a variety of settings.
Key Responsibilities- Provide comprehensive Annual Wellness Exams for Medicare Advantage patients in a clinical setting
- Formulate lists of current and past medical conditions using clinical knowledge and judgment
- Communicate findings to inform Primary Care Physicians of potential gaps in care
- Educate patients on disease processes, medication, and compliance
- Collaborate with healthcare teams to develop and implement value-based care models
- Utah Nurse Practitioner or Physician Assistant license and national certification
- Active DEA license with prescriptive authority
- Proficient in assessment and patient education
- Ability to work autonomously
- 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
- Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
- Knowledge of value-based model medicine, including HCCs and Medicare HEDIS/Stars measures
