About the Role:
Abode Care Partners is seeking a compassionate and skilled Home Healthcare Nurse Practitioner to provide personalized medical care to seniors and patients with complex needs in their homes or assisted living facilities. The ideal candidate will have a strong foundation in evidence-based medical care, excellent communication skills, and the ability to work independently.

Responsibilities:
Performs home visits to assess patient needs and develop comprehensive care plans
Examines and treats chronic and urgent episodic illnesses, minor injuries, and other conditions
Orders and interprets laboratory and diagnostic testing as needed
Collaborates with caregivers and other healthcare professionals to ensure seamless care delivery
Provides referrals to other healthcare practitioners when necessary
Ensures accurate and timely documentation of patient records

Requirements:
A valid state Nurse Practitioner license
Current National Board Certification for Nurse Practitioners
Active DEA licensure
Mastery's degree in Nursing (MSN) from an accredited institution
At least 5 years of nursing experience, with at least 2 years in a nurse practitioner role
Strong interpersonal and communication skills
Excellent time management skills and attention to detail
Demonstrated proficiency in electronic medical record systems and Microsoft Office applications
Licensed driver with valid automobile insurance

Compensation:
$100,000 - $120,000 per year, depending on performance and experience
