Job Title: Case Manager - Service Coordinator
">About the Role: We are seeking a skilled and compassionate Case Manager - Service Coordinator to join our team at Amcord Care. As a Service Coordinator, you will play a vital role in identifying, coordinating, and facilitating supportive services and community resources for elderly and/or disabled participants.
">Key Responsibilities:
">- ">
- Attend and complete orientation training and ongoing annual training (remote);">
- Manage an active caseload of 70-80 participants, monitoring and evaluating options and services to meet their health needs;">
- Complete person-centered assessments and assist members in obtaining HCBS services that support independent living;">
- Inform and assist members with gaining access to needed LTSS and Medical Assistance services;">
- Conduct monthly monitoring telephone calls and develop and modify service plans annually or more frequently as needed;">
- Maintain ongoing communication internally and with caseload;">
- Notify members of their right to choose any willing and qualified provider to provide services on their service plan;">
- Participate in remediation activities;">
- Responsible for maintaining confidentiality and following Amcord Care policies and procedures for document maintenance, records, and employees' rights;">
- Willingness and ability to work in the field while also working independently with integrity in a virtual setting.">
Requirements:

- Bachelor's degree in social work, psychology, or related field;">
- OR at least 3+ years of experience in a social service or healthcare-related setting;">
- Reliable transportation is required;">
- Bilingual preferred but not required.">
Salary: $20 per hour; approximately $41,600 per year, based on a 40-hour workweek.
