Key Responsibilities- Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
- Provide direct patient care, including administering medications, wound care, and other treatments as needed.
- Collaborate with the interdisciplinary team to ensure comprehensive care and coordination of services.
- Conduct regular assessments and evaluations to identify patient needs and make recommendations for care.
- Document patient information and progress in a timely and accurate manner.
- Graduate of a school of nursing.
- Washington Registered Nurse License.
- Provider BLS - American Heart Association.
- 2 years of Nursing experience.
- 1 year of Hospice, home health, skilled nursing facility, medical/surgical, or oncology experience.
