At Naven Health, we are seeking a highly skilled and compassionate Home Infusion Nurse to join our team. As a key member of our nursing network, you will play a vital role in delivering exceptional infusion care to patients in the comfort of their own homes.

Job Summary:

The Home Infusion Nurse is responsible for coordinating and providing direct infusion care to patients, ensuring their safety and well-being while providing continuity of care in compliance with the physician's plan of care. This role requires a strong commitment to patient-centered care, effective communication, and collaboration with the healthcare team.

Key Responsibilities:
  • Plan, provide, and document professional nursing care for patients in accordance with physician orders and established policies and procedures.
  • Assess patient needs and physical status through health data assessment and patient interview, and re-evaluate patient needs through physical assessment and response to care.
  • Provide effective and safe teaching using a patient-centered care approach, modifying teaching modalities to fit patient/family learning style to achieve independence with prescribed therapy and care needs.
  • Practice effective and timely communication with all members of the treatment team, including Naven Health's Clinical team, medical providers, patients, and families/caregivers, to ensure continuity of care.
  • Coordinate discharge planning, including preparing discharge summaries with patient instructions, and ensure that patient care needs and progress towards goals are documented.
Requirements:
  • Graduate of an accredited School of Nursing.
  • Current active and unrestricted registered nurse license.
  • At least 2 years of clinical experience working as a RN in an area that requires training and hands-on experience in infusion therapies, or at least 1 year of clinical experience as a RN working in an area noted above, with an IV nursing certification.
  • Current certification in Basic Life Support for Health Care Professionals.
Preferred Qualifications:
  • Bachelor of Science in Nursing from an accredited school of nursing.
  • CRNI, IgCN, OCN highly preferred.
Benefits:
  • Competitive salary range: $36.17-$60.28.
  • 401k plan.
