Position Type: Full-Time & Per Diem Hours:

Monday-Friday 8AM to 5PM (Alternating Weekends) Overview:

The Hospice Aide plays a crucial role in delivering compassionate care to hospice patients and their families, primarily under the guidance of a Registered Nurse or Physician.

Key Responsibilities:
  • Provide personal care and home maintenance tasks as outlined in a care plan developed by a healthcare professional.
  • Conduct visits based on assessed patient needs, adhering to scheduled time frames.
  • Facilitate effective communication with the Interdisciplinary Team.
  • Notify patients of visit times and document visits in the patient’s home.
  • Report to the Clinical Care Manager in cases where a patient declines service.
  • Inform the Clinical Care Manager if there is no response at the door before leaving the patient’s residence.

Embrace feedback from quality assurance evaluations and implement necessary improvements to ensure high standards of care and customer service.

  • Assist patients with mobility, following established nursing protocols.
  • Report any changes in patient conditions to the Registered Nurse.
  • Remain adaptable to schedule modifications as patient needs evolve.
  • Accurately document services rendered.
  • Engage in Interdisciplinary Team meetings.
  • Support the orientation of new Hospice Aides under the supervision of the Clinical Care Manager.

Documentation: Ensure precise and timely documentation of services, utilizing required technology as needed.

Additional Responsibilities:
  • Exhibit patience and hospitality while assisting patients, families, healthcare providers, and colleagues in challenging situations.
  • Demonstrate essential skills to promote patient independence through verbal and physical instruction.
  • Maintain composure and effectiveness in high-stress environments.
  • Keep all necessary licenses, certifications, and skills updated as mandated.
  • Adhere to all organizational policies and relevant laws and regulations.
  • Fulfill additional duties as assigned by the healthcare team.

Supervisory Role: May assume interim leadership responsibilities as required.

Physical Demands:

Must be capable of lifting and/or moving up to 50 pounds and pushing/pulling over 250 pounds, as well as standing for extended periods and climbing stairs or ladders.

Requirements:

  • Active Certified Nursing Assistant certification in the state of Florida.
  • Current CPR certification.
