Position Type: Full-Time & Per Diem Hours: Monday-Friday 8AM to 5PM (Alternating Weekends)

Overview

The Hospice Aide plays a crucial role in delivering compassionate care to hospice patients and their families, working closely with or under the guidance of a Registered Nurse or Physician.

Key Responsibilities
  • Provide personal care and home support services as directed by a healthcare professional.
  • Conduct visits based on assessed needs, adhering to established schedules.
  • Facilitate effective communication with the Interdisciplinary Team.
  • Notify patients of visit times through phone calls.
  • Document visits in the patient’s sign-in log during each appointment.
  • Inform the Clinical Care Manager if a patient declines services.
  • Contact the Clinical Care Manager before leaving if there is no response from the patient/family.
  • Accept and implement feedback from quality assurance calls to enhance service quality.
  • Assist patients with mobility according to accepted nursing practices.
  • Report any changes in patient conditions to the Registered Nurse.
  • Adapt to schedule changes to accommodate patient and family needs.
  • Accurately record services rendered.
  • Participate in Interdisciplinary Team meetings.
  • Support the training of new Hospice Aides under the supervision of the Clinical Care Manager.
  • Ensure timely and precise documentation, both written and through required technology.
Additional Responsibilities
  • Exhibit hospitality and patience while assisting patients, families, healthcare providers, and colleagues in challenging situations.
  • Demonstrate basic skills to promote patient independence through verbal instructions.
  • Maintain composure and effectiveness in high-stress scenarios.
  • Keep all necessary licenses, certifications, and skills updated as required.
  • Adhere to all policies and regulations at local, state, and federal levels.
  • Contribute to the healthcare team as needed.
  • Perform other assigned duties as required.
Supervisory Role
  • May assume interim leadership responsibilities based on departmental needs.
Physical Demands
  • Ability to lift and/or move up to 50 pounds and push/pull over 250 pounds, as well as walk, climb stairs or ladders, and stand for extended periods.

Qualifications
  • Active Certified Nursing Assistant certification in the state of Florida.
  • Current CPR certification.
  • Minimum of one year of relevant experience.
  • Proficient in clinical and office software.
  • Ability to read, write, and comprehend the English language.
