Key Responsibilities:- Assess and monitor the medical needs of patients/significant others
- Provide quality medical services to patients in a cost-effective and appropriate manner

- Communicate with other members of the healthcare team to ensure continuity of care and coordination of services
- Participate in ongoing process improvement activities through the development/application of clinical paths and other related activities
