We are seeking a skilled Certified Wound Ostomy Nurse to join our Home Health team. As a Wound Ostomy RN, you will provide high-quality care to patients with ostomies, draining wounds, fistulas, pressure ulcers, acute and/or chronic wounds, and incontinence in the comfort of their own homes.

Responsibilities
  • Coordinate and provide care to patients with complex wound and ostomy needs
  • Develop and implement individualized care plans to promote wound healing and ostomy management
  • Provide education to patients and their caregivers on wound and ostomy care, including ostomy management, wound dressing changes, and skin care
  • Collaborate with healthcare team members to ensure comprehensive care and coordination
  • Perform related supply management and documentation tasks
Requirements
  • Bachelor's Degree in Nursing
  • California Registered Nurse License
  • National Provider BLS - American Heart Association
  • 1 year of nursing experience, preferably in home health or wound care
