Thrive Skilled Pediatric Care LLC is seeking a dedicated and compassionate Pediatric Home Health Nurse to provide high-quality clinical home care to medically fragile children. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the Role:
- Provide one-to-one care to pediatric patients in their homes, promoting their physical, emotional, and developmental growth.
- Develop and implement individualized care plans in collaboration with our interdisciplinary team.
- Conduct thorough assessments, identify patient needs, and coordinate care with other healthcare professionals.
- Administer medications, perform treatments, and provide education to patients and their families.
- Collaborate with our Social Responsibility Committee to advocate for the needs of our patients and their families.
Requirements:
- Current nursing license and current CPR certification.
- Experience working with pediatric patients, preferably in a home health setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a focus on providing exceptional patient care.
