Job Summary

As a Physical Therapist Assistant with Choice Health at Home, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. We are seeking a skilled and compassionate professional to join our team and provide rehabilitative programs that improve mobility, relieve pain, and enhance overall well-being.

Key Responsibilities
  • Conduct thorough initial exams to evaluate patient needs and develop personalized treatment plans.
  • Implement and document individually designed programs to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction.
  • Evaluate treatment effects and adjust plans as needed to achieve maximum benefit.
  • Apply infection control and universal precautions to ensure a safe and healthy environment for patients.
  • Ensure continuity of quality patient care through accurate documentation and effective communication with patients, families, and healthcare professionals.
  • Promote agency philosophy and administrative policies to maintain a positive and productive work environment.
Requirements
  • Successful completion of an accredited physical therapy program and state licensure exam.
  • Valid Physical Therapy license.
  • Valid state driver's license.
  • 1 year of home health experience preferred.
  • AHA CPR certification.
  • Valid Driver's License.
  • Valid Auto Insurance.
  • HomeCare HomeBase experience preferred.
Physical Demands
  • Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
  • Ability to lift, carry, pull, or push up to 100 lbs.
