The Renal Care Nurse is responsible for delivering patient care in accordance with the established treatment protocols; actively participates in and documents the progress of each patient's care plan; provides ongoing education; and communicates effectively regarding individual and unit processes.
The Renal Care Nurse is accountable for ensuring that all care delivered and tasks assigned are executed in compliance with the clinic's policies and procedures, as well as state and federal regulations.
- Utilizes assessment, planning, implementation, and evaluation to address individual patient and facility requirements, adhering to all relevant policies and regulations.
- Sets up hemodialysis equipment and initiates, monitors, and concludes dialysis treatments in accordance with regulatory standards. Observes patient vital signs before, during, and after treatment, intervening in non-routine or at-risk situations to ensure prescribed treatments are delivered.
- Records all relevant facility and patient information, assessments, care plans, and evaluations in the medical record in accordance with established policies and procedures.
- Applies pharmacological principles to the prescribed care plan for patients. Accurately manages, prepares, administers, and documents all prescribed medications, ensuring compliance with applicable regulatory standards.
- Demonstrates effective delegation and supervision within the clinical environment. Assigns duties in accordance with state regulatory requirements and provides timely patient data to the supervising associate to ensure patient and environmental safety.
- Evaluates the health education needs of patients and their families, providing instruction focused on health maintenance, disease prevention, and self-care strategies. Mobilizes appropriate resources to effectively meet those needs and serves as an educational resource.
