About the Role:

The Tufts Medical Center is seeking a skilled and compassionate Registered Nurse to join our team as a Senior Nurse Practitioner - Patient Care Coordinator. This role requires a seasoned professional with expertise in nursing care, leadership skills, and a passion for delivering exceptional patient experiences.

Responsibilities:

  • Lead and coordinate patient care teams to ensure high-quality, efficient, and effective care delivery.
  • Collaborate with multidisciplinary teams to develop and implement comprehensive care plans that meet individual patient needs.
  • Provide clinical guidance and mentorship to junior staff members, promoting professional development and growth.
  • Conduct regular patient assessments, identify areas for improvement, and implement changes to enhance patient outcomes.
  • Develop and maintain relationships with patients, families, and healthcare providers to ensure seamless communication and care coordination.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in patient care.

Requirements:

  • Bachelor's degree in Nursing required; Master's degree or higher preferred.
  • Current RN licensure and BLS certification required; Advanced certifications (e.g., CCRN, CNOR) a plus.
  • Minimum 3 years of experience in a similar role, with a strong focus on leadership, collaboration, and patient-centered care.
  • Excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a fast-paced environment.
