We are seeking a skilled and compassionate Senior Nurse Practitioner to join our PACE Program team at East Boston Neighborhood Health Center. Job Summary

The Senior Nurse Practitioner will be responsible for the general oversight, assessment, planning, and construction of the nursing care delivered to participants in our PACE Program. This includes assessing patients through obtaining history, performing appropriate tests, making or collecting observations from participants and caregivers, and consulting with other members of the Interdisciplinary Team (IDT).

Responsibilities
  • Teach patients and families health care regimens, explain disease and wellness, test results, medications, diet, exercise, and other aspects of the plan of care.
  • Safely and effectively administer medications, tests, and treatments as ordered, manage refills of all patient medications with provider authorization.
  • Provide and coordinate health teaching required by individuals, families, and groups to maintain optimal possible level of health.
  • Complete biannual and status change nursing assessments for assigned participants in accordance with Care Planning Policy, documenting problems, goals, and interventions in the participant's care plan.
Qualifications and Requirements
  • Baccalaureate degree in nursing preferred, current Massachusetts license in nursing, BLS certification required.
  • Minimum of 2 years experience in ambulatory, home health, hospital, or sub-acute setting preferred, equivalent training or related experience considered.
  • A minimum of one year experience working with a frail or elderly population, prior experience in IDT setting a plus.
Salary and Benefits

This is a full-time position with a competitive salary range of $80,000-$110,000 per year, depending on experience.
