About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at the Good Samaritan Society. As an LPN, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary care team to ensure the best possible outcomes.

Key Responsibilities
  • Provide professional nursing care to residents of all ages in long-term care settings, under the supervision of a registered nurse, advanced practice provider, or physician.
  • Assess residents' physical and psychosocial needs, develop and implement individualized care plans, and provide education to residents and their families.
  • Administer medications and treatments as ordered, and document resident interactions and outcomes.
  • Collaborate with healthcare professionals to ensure seamless care delivery and communicate effectively with residents, families, and colleagues.
  • Counsel residents, family members, and/or resident advocates on prevention and treatment plans, and develop self-care programs.
Requirements
  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
  • Associate degree in practical nursing is preferred.
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
  • Obtains and subsequently maintains required department-specific competencies and certifications.
