We are seeking a highly skilled Care Coordinator to join our team at MaineHealth Behavioral Health Autism and Developmental Disorders Portland.
This is a full-time day shift position that requires a Bachelor's Degree from an accredited institution with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, or a closely related field.
Key Responsibilities:
- Provide comprehensive care coordination services to patients and families.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare professionals to ensure seamless care transitions.
