The Staff RN-Ambulatory Clinics is a key member of the interprofessional team, providing patient-centered care and working collaboratively to achieve optimal health outcomes. This role involves systematic data collection and assessment to develop, implement, and evaluate patient care plans. The Staff RN-Ambulatory Clinics utilizes evidence-based practice to inform clinical decisions and drive individual and population health initiatives.
Key Responsibilities- Provide patient care coordination and direct patient care services
- Develop and implement patient education programs
- Participate in telephone triage and eVisits
- Perform and assist with minor in-clinic procedures
- Float to other areas as needed to support operational needs
- Clinical experience and/or interest in patient population care
- At least 1 year of relevant nursing experience preferred
- Nursing licensure required
- Bachelor's Degree in Nursing preferred
- Current state of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating state in the NLC
- American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA-approved equivalent
