Nurse Case Manager

Job Summary:
As a skilled Registered Nurse with SAFREST, you will provide direct and indirect primary nursing care to assigned participants in both clinic and home-based environments. You will work under the direct and indirect supervision of the Program Director or physician extender, applying advanced nursing knowledge and skills independently.
Key Responsibilities:
- Manage complex clinical situations and supervise both licensed and non-licensed personnel.
- Perform assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures.
- Build a trusting relationship with each participant/family/caregiver by being available, listening, communicating, and educating via every available forum.
Requirements:
- Must be a Registered Nurse in the state of Pennsylvania.
- Must have a valid driver's license and a reliable vehicle.
What We Offer:
- Mileage reimbursement.
- A caseload that allows for autonomy and the ability to work at your own pace.
- A Monday-to-Friday schedule from 8 a.m. to 4:30 p.m.
