Advanced Practice Registered Nurse

This role is integral to the healthcare delivery team. The individual in this position will deliver primary healthcare services, which encompass evaluating and managing patients within the clinic while adhering to established clinical guidelines. This role is empowered to make autonomous decisions regarding the advanced clinical needs of patients and families, in collaboration with supervising physicians.
KEY RESPONSIBILITIES
- Formulates and oversees the medical care plan for patients, developed collaboratively with supervising physicians.
- Delivers thorough outpatient care, including health assessments and management of medical conditions, with follow-up on identified issues.
- Manages both acute and chronic conditions, coordinating care plans with behavioral health providers when necessary.
- Facilitates appropriate referrals to internal departments or external agencies for additional services as required.
- Prescribes medications as necessary for the treatment of various conditions.
- Provides clinical guidance to other healthcare providers within the organization as needed.
- Ensures accurate, timely, and comprehensive documentation in medical records.
- Adheres to clinic protocols and professional regulatory standards.
- Communicates effectively with clinical support staff regarding patient follow-ups, including informing patients of test results.
- Maintains clear communication with patients, guardians, and colleagues.
- Participates in an on-call rotation for after-hours patient care as assigned.
- Engages in quality improvement initiatives.
- Evaluates the costs and benefits of potential clinical actions.
- Supervises healthcare students as assigned.
- Possession of a full medical practice license in the State of Kansas or Oklahoma, depending on the work location.
- Graduation from an accredited advanced nurse practitioner program.
- Preferred one year of experience in a clinical environment.
- Strong orientation towards community and public health is preferred.
- Commitment to serving underrepresented populations.
- If working in a Walk-In Care facility, must obtain DOT Certification within the first month of employment and recertify every decade.
- Demonstrates cultural sensitivity to a diverse patient and employee population.
- Possesses knowledge of effective patient care practices.
- Communicates through appropriate channels, utilizing the proper chain of command for patient concerns.
- Ability to remain calm and effective in emergency situations.
- Proficient in computer use and navigating Electronic Health Records.
- Maintains positive interpersonal relationships with colleagues and other healthcare team members.
- Delivers customer service aligned with the organization's mission.
- Exhibits courtesy and respect in interactions with patients and their families.
- Ensures patient confidentiality in compliance with organizational policies and HIPAA regulations.
- Capable of performing basic mathematical operations in various units of measure.
In this role, the employee is regularly required to sit or stand, use hands for various tasks, reach with arms, and communicate effectively. The position may require occasional lifting or moving of items up to 10 pounds. Specific vision abilities necessary for this role include close vision, peripheral vision, depth perception, and the ability to adjust focus.
