Advanced Practice Registered Nurse

This role is integral to the healthcare delivery team. The individual in this position will deliver primary healthcare services, which encompass evaluating and managing patients within the clinic while adhering to established clinical guidelines. This role is empowered to make autonomous decisions regarding the advanced clinical needs of patients and families, in accordance with collaborative practice agreements with supervising physicians.
KEY RESPONSIBILITIES
- Formulates and oversees the medical care plan for patients, based on a collaborative agreement established with supervising physicians.
- Delivers thorough outpatient care, including health assessments and treatment for various medical conditions, ensuring follow-up on identified issues.
- Manages both acute and chronic conditions, coordinating care plans with behavioral health providers when necessary.
- Facilitates appropriate referrals to internal and external resources for additional services as required.
- Prescribes medications for the treatment of medical conditions as deemed necessary.
- Provides clinical guidance to other healthcare providers within the organization as needed.
- Ensures accurate, timely, and comprehensive documentation in medical records.
- Adheres to clinic protocols and professional regulatory standards.
- Maintains effective communication with clinical support staff, including informing patients of test results through appropriate channels.
- Communicates clearly and effectively with patients, guardians, and colleagues.
- Participates in an on-call rotation for after-hours patient care as assigned.
- Engages in quality improvement initiatives.
- Evaluates the cost-effectiveness of potential clinical actions.
- Supervises healthcare students as assigned.
- Possession of full licensure to practice as a nurse practitioner in the relevant state.
- Graduation from an accredited advanced nurse practitioner program.
- Preferred experience of at least one year in a clinical environment.
- Preferred strong orientation towards community and public health.
- Dedication to serving underserved populations.
- If working in a Walk-In Care facility, must obtain necessary certifications as required.
- Demonstrates cultural and social sensitivity appropriate for a diverse patient population.
- Exhibits knowledge of effective patient care practices.
- Communicates through established channels and follows the proper chain of command for patient concerns.
- Ability to manage emergency situations with composure.
- Proficient in using electronic health record systems.
- Maintains positive interpersonal relationships with colleagues and healthcare team members.
- Delivers customer service aligned with the organization's mission.
- Interacts courteously and respectfully with patients and their families.
- Upholds patient confidentiality in accordance with organizational policies and HIPAA regulations.
- Capable of performing basic mathematical calculations relevant to healthcare.
In the course of fulfilling job responsibilities, the employee is frequently required to sit or stand, utilize hands for various tasks, reach with arms, and communicate verbally. The role may involve occasional lifting or moving of light objects. Specific vision capabilities necessary for this position include close vision, peripheral vision, depth perception, and the ability to adjust focus.
