Key Responsibilities
  • Develop and maintain a community outreach strategy to target high-risk populations, ensuring effective partnerships with clinicians, administrators, patients, and community organizations.
  • Manage quality control for all screenings and community outreach activities, ensuring compliance with established protocols.
  • Analyze data from community screenings and outreach activities to inform program development, identifying areas for improvement and opportunities for growth.
  • Schedule and attend community outreach activities, screenings, and education programs, providing expert guidance and support to patients and community members.
  • Provide direct patient counseling on health-related issues, connecting patients to relevant resources and services to promote optimal health outcomes.
  • Collaborate with clinicians, administrators, and community organizations to facilitate partnerships and drive health outcomes, leveraging expertise and resources to achieve shared goals.
Requirements
  • Bachelor's degree in a related field, such as health education or a related field.
  • At least 5 years of experience in a clinical or administrative role in healthcare, with a proven track record of success in community outreach and education programs.
  • Proven ability to write program curriculum and speak on various healthcare topics, demonstrating expertise and passion for health education.
  • Experience in community outreach and education programs, with a strong understanding of the needs and challenges faced by underserved populations.
Preferred Qualifications
  • Health education credentials or clinical experience, with a strong foundation in health education principles and practices.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with diverse stakeholders.
  • Proficiency in Microsoft Office software, including Outlook, Excel, PowerPoint, and Word, with experience in electronic health records a plus.
