Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Chestnut Woods Rehabilitation & Healthcare Center. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our staff to ensure the best possible outcomes for our residents.

Responsibilities
  • Provide patient care in accordance with established nursing standards and protocols.
  • Develop and implement individualized care plans in collaboration with our Nurse Manager and healthcare team.
  • Administer medications and treatments as prescribed by physicians and in accordance with our policies and procedures.
  • Monitor residents' conditions and report any changes or concerns to our Nurse Manager and healthcare team.
  • Document patient care and treatment in accordance with our policies and procedures.
Requirements
  • Must possess an active, unencumbered license to practice as an LPN in Massachusetts.
  • Covid vaccination required.
