Licensed Practical Nurse

Bridgewell is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a residential program. As an LPN, you will provide health services and direct care to individuals with disabilities, working collaboratively with our multidisciplinary team to ensure the highest level of care.
Key Responsibilities- Provide direct specialized nursing and other day-to-day care to individuals
- Document health care services following appropriate state regulations, nursing standards, and Massachusetts LPN license requirements
- Act as a liaison with community health care providers, monitoring medical records, and medication administration
- Provide in-service education to individuals and/or staff on health issues
- Maintain knowledge of the Department of Public Health Medication Administration Program (MAP) current policies and procedures
- Graduation from an accredited and approved licensed practical nurse program
- Current Massachusetts Nursing license
- Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting
- Experience working with adults with DD and behaviors is a plus
