Key Responsibilities- Conduct Home Visits: Meet with individuals and their families in their homes to assess their needs and provide recommendations for care and support.
- Schedule Follow-up Visits: Schedule timely follow-up visits to ensure that the recommended care and support are being provided.
- Document Visits: Complete all necessary documentation for home visits, including assessment reports and follow-up visit notes.
- Communicate with Team Members: Relay information to coordinators and support services managers to ensure seamless communication and coordination of care.
- Report Incidents: Report any incidents or concerns to coordinators and program directors.
- Enter Data: Enter home visit update information into the correct systems.
- Maintain Communication: Maintain ongoing communication with individuals, coordinators, and program directors to ensure that their needs are being met.
- Education: High school diploma or equivalent, with two years of supervisory experience or one year of supervisory experience with one year of college. Bachelor's degree preferred.
- Valid Driver's License: Valid driver's license from the state of residence with a satisfactory driving record.
- Excellent Communication Skills: Excellent customer service and communication skills, with the ability to function well under pressure and manage a large amount of information.
- Direct Care Experience: Direct care and supervisory experience working with disadvantaged populations.
- Technical Skills: Knowledge and skills in information systems.
- Team Player: Skilled at assisting others and accepting direction.
