Home Health RN Case Manager

We are seeking a dedicated and compassionate Home Health RN Case Manager to join our team at Quality Home Health Care. As a Home Health RN Case Manager, you will play a vital role in providing high-quality patient care and coordinating services to meet the needs of individuals and their families in the comfort of their own homes.
Responsibilities:- Conduct initial assessments of patients and families to determine home care needs, including physical assessments and histories of current and previous illnesses.
- Develop and implement care plans that establish goals based on nursing diagnoses, incorporating therapeutic, preventative, and rehabilitative nursing actions.
- Initiate and revise care plans as patient status and needs change.
- Provide direct patient care as defined in the State Nurse Practice Act.
- Counsel patients and families on meeting nursing and related needs.
- Provide education and healthcare instructions to patients as appropriate per assessment and plan of care.
- Identify discharge planning needs as part of care plan development and implement prior to discharge of the patient.
- Act as Case Manager and coordinate patient care for assigned caseload.
- Contribute to care coordination and patient-centered care as part of an interdisciplinary clinical care team.
- Associate's or Bachelor's degree in Nursing with one year of home health care experience preferred.
- AZ Nursing licensure in good standing.
- Current CPR/BLS certification.
- Fingerprint clearance card.
- TB test results within the last year.
- AZ Driver's license and proof of insurance with reliable transportation.
- Excellent observation, verbal, and written communication and problem-solving skills.
