**Job Summary**

We are seeking a highly skilled Referral Manager to join our team of healthcare professionals. The successful candidate will be responsible for coordinating referrals and ensuring seamless communication between healthcare providers.

**Key Responsibilities**

• Coordinate referrals and communicate with healthcare providers to ensure timely and effective care

• Develop and maintain relationships with healthcare providers and community organizations

• Ensure compliance with regulatory requirements and industry standards

• Collaborate with the healthcare team to improve patient outcomes and satisfaction

• Participate in quality improvement initiatives to enhance patient care and services
