Job Summary

We are seeking a highly skilled Referral Manager to join our team. The Referral Manager will be responsible for coordinating patient referrals and ensuring seamless transitions between healthcare providers.

Key Responsibilities

• Coordinate patient referrals and communicate with healthcare providers to ensure timely and effective care.
• Manage and maintain accurate and up-to-date patient records.
• Develop and implement effective referral processes to improve patient outcomes.
• Collaborate with healthcare teams to ensure high-quality care and patient satisfaction.
• Provide exceptional customer service to patients, families, and healthcare providers.
