Job Summary

We are seeking a highly skilled Physical Therapist to join our team in providing community-based outpatient services to patients in the Somerset, NJ area. As a Physical Therapist with Career Tree Network, you will have the opportunity to work with a well-balanced mix of patients in the homes of older adults, providing evidence-based care and making a meaningful impact on their lives.

Key Responsibilities
  • Provide 12 hours of direct patient care per week, with a minimum of 3 weekdays of availability
  • Work with a multidisciplinary therapy team to develop and implement treatment plans
  • Conduct home-based assessments and treatments, utilizing a patient-centered approach
  • Collaborate with patients, families, and caregivers to ensure seamless care transitions
  • Stay up-to-date with industry developments and best practices in physical therapy
Requirements
  • Graduation as a Doctor of Physical Therapy (or equivalent standing)
  • Eligibility for New Jersey state licensure as a Physical Therapist
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Passion for providing high-quality, patient-centered care
