Palliative Care RN Coordinator

OU Health is seeking a skilled RN Palliative Care Coordinator to join our team. As a key member of our Palliative Care Team, you will provide professional nursing-related healthcare in a medical specialty, assessing, planning, implementing, and evaluating nursing care in a designated patient population or specialty.
Responsibilities:- Assist physicians during examinations as needed, ensuring all information is available and accompanying physicians on rounds to stay informed of special orders.
- Make follow-up patient appointments, answer routine questions, and provide educational information to patients and families.
- Collaborate with healthcare professionals to provide efficient quality patient care, evaluating and improving teaching and nursing practices, and recommending modifications.
- Assess and monitor patient response to procedures and treatments, reporting patient condition and reaction to tests, drugs, treatments, and any significant incidents to the physician.
- Counsel patients and families about death, side effects of medication, therapy, and other relevant topics.
- Participate in and contribute to research activities related to the area of specialization, participating in conferences pertaining to the research area.
- Intake of new inpatient consults
- Schedule inpatient appointments
- Coordinate care conferences
- Manage comfort cart
- Hospice referrals and enrollment
- Coordinate/complete discharge follow-up calls
- Attend tele-health visits (once monthly)
- Facilitate transitions from perinatal palliative care to general palliative care service
- Facilitate transitions from OCH to BCHC and vice versa
- Facilitate enrollment into bereavement program
- Triage of oncology consults
- Intake of oncology patients
- Coordination of oncology clinic schedule with palliative care visits
- Graduate from an accredited school of nursing required.
- Develop and maintain educational materials for patients/families
- Manage/maintain palliative care patient database
- Quality improvement initiatives
- Current RN License issued by the Oklahoma State Board of Nursing, or a current multistate compact RN License (eNLC)
- Current Basic Life Support issued by the American Heart Association required upon hire
