Job Overview

At Volunteers of America Los Angeles, we are seeking a skilled Senior Case Manager II to join our team. This is a full-time position that requires a combination of administrative and field work. The ideal candidate will have a strong background in case management, excellent communication skills, and the ability to work effectively with clients and colleagues.

About the Job

The Senior Case Manager II will serve as the subject matter expert for the case management team. In this role, you will mentor junior case managers, provide guidance and support, and ensure that all services delivered by the team meet the needs of clients. This includes conducting comprehensive screenings and assessments, developing service plans, and coordinating referrals to community resources.

Key Responsibilities
  • Case Management: Conduct comprehensive screenings and assessments with clients to collect functional, environmental, financial, employment, housing, educational, and health information. Develop relevant service plans and schedule appointments as needed.
  • Mentorship and Training: Mentor junior case managers on case management principles, best practices, and program requirements.
  • Team Collaboration: Work collaboratively with the case management team to achieve shared goals and objectives.
  • Client Support: Provide ongoing case conferencing, counseling, and advocacy to clients and families as needed.
Requirements and Qualifications

To be successful in this role, you will need:

  • Education: Bachelor's degree from an accredited institution in social sciences or family therapy.
  • Experience: Two years of demonstrated case management experience with high needs/high intensity populations.
  • Skills: Excellent communication and interpersonal skills, ability to work effectively in a fast-paced environment, and proficiency in MS Office and HMIS navigation.
