Patient-Focused Physical Therapist

We are seeking a dedicated and skilled Physical Therapist to join our team at Care IV Home Health. As a member of our organization, you will have the opportunity to work in a supportive and flexible environment that prioritizes patient care and professional growth.

Key Responsibilities:
- Conduct home visits to assess and treat patients in their residences or assisted living facilities.
- Develop individualized physical therapy plans tailored to each patient's needs.
- Monitor and document patient progress, including admissions, discharges, and OASIS assessments.
- Collaborate with the Director and other team members to ensure comprehensive care.
Qualifications:
- Active Physical Therapy license in Arkansas or registered in the state.
- Minimum of three professional references available upon request.
- Previous experience in home health is advantageous but not mandatory.
