About the Role

We are seeking a skilled and dedicated Advanced Healthcare Professional to join our team at ReviveRX in Portland, ME. This is an excellent opportunity for a talented Nurse Practitioner to make a meaningful impact in the field of healthcare.

Job Responsibilities
  • Conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic tests.
  • Develop and implement personalized care plans tailored to each patient's needs and wellness goals.
  • Provide preventive care services, such as health screenings, vaccinations, and lifestyle counseling.
  • Manage chronic conditions like diabetes, hypertension, and obesity through customized treatment plans.
  • Prescribe medications and treatments as necessary, adhering to established protocols and guidelines.
  • Educate patients on effective health maintenance, disease prevention, and wellness strategies.
  • Collaborate with other healthcare professionals to ensure coordinated and comprehensive care.
  • Maintain accurate and up-to-date patient records in compliance with HIPAA regulations.
  • Participate in ongoing professional development and education activities to stay current with best practices and advancements in the field.
Requirements
  • A Master's or Doctorate degree in Nursing from an accredited program.
  • Current certification and licensure as a Nurse Practitioner in the state of Maine.
  • A minimum of 4 years of experience as a Nurse Practitioner, preferably in a wellness or preventive care setting.
  • Strong clinical skills with the ability to perform comprehensive patient assessments.
  • Excellent communication and interpersonal skills, with a patient-centered approach to care.
  • Ability to work independently as well as part of a multidisciplinary team.
  • Proficiency in electronic health record (EHR) systems.
  • Commitment to ongoing professional development and education.
Compensation and Benefits

The estimated annual salary for this role is $120,000, based on industry standards and location.
