About the RoleWe are seeking a highly skilled Clinical Pharmacist Specialist to join our team in the outpatient clinic, focusing on heart and kidney transplant patients. As a key member of our team, you will work closely with physicians and other healthcare professionals to provide comprehensive pharmaceutical care and quality assurance programs.
Responsibilities- Evaluate and recommend appropriate drug therapy to achieve optimal patient outcomes.
- Participate in the development, provision, and evaluation of comprehensive pharmaceutical care and quality assurance programs.
- Serve as a resource to the pharmacy staff and other hospital departments.
- Assess the overall status of the patient's health condition and evaluates medication profile for optimal patient outcomes.
- Monitor therapy and goals of care, documenting all activities and communication as appropriate.
- Participate in professional development and research activities.
- Contribute to performance improvement, clinical effectiveness, drug use evaluation, and HSO/BJC committee activities.
- Participate in the education and training of pharmacy staff, residents, and students, and serve as a role model and mentor.
- Participate in the identification and implementation of targeted drug cost reduction programs.
- Doctorate in Pharmacy.
- Medication Therapy Services in MO.
- RPh.
- 2-5 years of experience in a related field.
