Job Summary

Patient Care Management is a critical role at Upstate Medical University, requiring a skilled and compassionate healthcare professional to provide high-quality care to patients. As a Nurse Practitioner, you will work closely with physicians to deliver exceptional patient outcomes.

Key Responsibilities
  • Conduct thorough health assessments, including health histories and physical examinations, to identify patient needs and develop effective care plans.
  • Initiate and perform diagnostic procedures, laboratory examinations, and other tests as necessary to inform patient care decisions.
  • Collaborate with physicians and other healthcare professionals to ensure seamless patient care and continuity of service.
  • Develop and implement treatment plans, including medication management, assistive devices, and other interventions to promote patient well-being.
  • Provide education, counseling, and support to patients and their families to empower them to manage their health effectively.
  • Participate in research projects and quality improvement initiatives to advance the field of nursing and improve patient outcomes.
Requirements
  • NYS licensed/registered NP or PA required, with NYS NP specialty area of certification consistent with practice.
  • NPI number and Medicare Enrollment with Medicare Identification Number (or PTAN) required, or eligibility for Medicare Enrollment within 6 months of hire.
  • Current BLS certification through the American Heart Association or the American Red Cross required.

  • Work experience in a healthcare setting, with a focus on patient care management and clinical practice.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work in a fast-paced environment, with a focus on delivering high-quality patient care and meeting productivity standards.

Salary: $92,000 - $147,004, depending on experience and qualifications.
