Family Engagement Coordinator

Position Title: Family Engagement Coordinator
This is a full-time role (10 months per year) associated with the Southern Seven Health Department. The position includes a comprehensive benefits package, which encompasses vacation, personal, and sick leave accrual, holidays, health and life insurance, and an employer-sponsored cafeteria plan. The Family Engagement Coordinator is responsible for providing case management services to families enrolled in the program.
Qualifications: A minimum of an Associate Degree in Social Work or a related field is required; a Bachelor’s Degree in Social Work and experience in family support is preferred.
Compensation: $17.55 - $21.92 per hour
Key Responsibilities:
- Facilitates family goal-setting through the Family Partnership Agreement.
- Assists families in acquiring health documentation and necessary follow-up care.
- Conducts home visits to support families.
- Records case notes and services in the program database.
- Maintains knowledge of community resources and refers families to appropriate services.
- Organizes outreach activities to familiarize families with the program.
- Completes face-to-face intakes, including income verification for applicants.
- Supports families in advocacy efforts, such as attending meetings related to educational plans.
- Leads monthly parent meetings at designated locations.
- Promotes engagement activities for parents, families, and the community.
- Performs additional duties as assigned to fulfill the program's requirements.
