Care Navigator

We are seeking a highly skilled and compassionate Care Navigator to join our team at Homeward Health. As a Care Navigator, you will play a vital role in helping our members achieve their best health by providing proactive outreach, building relationships, and connecting them with necessary services.
Key Responsibilities- Conduct telephonic, video, and in-person outreach to build relationships with members and connect them with medical, behavioral, and social services.
- Work with members and their providers to achieve their health goals and address gaps in care.
- Deeply understand the local community, geography, culture, healthcare ecosystem, and available resources to provide effective care.
- Collaborate with members' primary care providers and their teams to ensure cohesive care.
- Document all member encounters in the designated electronic platform in a timely manner.
- Attend regular staff meetings, trainings, and other meetings as requested.
- Completion of a Medical Assistant program from an accredited program or school.
- At least two years of experience in high-touch, patient-facing roles, preferably with seniors.
- Passion for delivering care in rural America and ability to persuade members to take actions that support their health.
- Expertise with the local community, geography, culture, healthcare ecosystem, and available resources.
- Previous experience with care navigation or community health work.
