We are seeking a compassionate and skilled Care Coordinator to join our team at Fenway Health, a leading provider of healthcare services to the LGBTQIA+ community. In this critical role, you will play a vital part in delivering high-quality care to patients while promoting a culture of inclusivity and respect.

About the Role:

As a Care Coordinator, you will be responsible for coordinating patient care across various settings, including ambulatory, hospital, and public health nursing. Your primary focus will be on ensuring seamless transitions between care providers, facilitating communication among healthcare teams, and advocating for patients' needs.

Key Responsibilities:
  • Patient Advocacy: Empathetically support patients and their families by addressing their concerns, preferences, and cultural needs.
  • Care Coordination: Collaborate with healthcare teams to develop and implement comprehensive care plans that meet patients' medical, social, and emotional needs.
  • Communication: Effectively communicate with patients, families, and healthcare professionals to ensure continuity of care.
  • Nurse Assessment and Triage: Conduct virtual, telephonic, or in-person nurse assessments and triage patients to determine the severity of their health concerns.
  • Documentation: Maintain accurate and timely documentation of patient information in electronic health records (EHRs).
Requirements:

To succeed in this role, you will need:

  • A degree in Nursing (Associate's degree required; Bachelor's degree preferred)
  • A current Massachusetts Registered Nurse (RN) license
  • Current CPR certification
  • Excellent interpersonal and communication skills
  • Experience working in ethnically, culturally, and racially diverse environments
  • An authentic interest in LGBTQIA+ and community health
  • Proficiency in computer software, such as MS products
