Job Summary

We are seeking a compassionate and dedicated Care Partner to join our team at The Chateau at Gardnerville Assisted Living and Memory Care. As a Care Partner, you will play a vital role in providing exceptional care and support to our residents, ensuring their physical, emotional, and social needs are met.

Key Responsibilities
  • Provide personalized care and support to residents, including assistance with daily living activities, medication management, and mobility assistance.
  • Develop and maintain strong relationships with residents, their families, and healthcare professionals to ensure seamless care coordination.
  • Participate in resident-centered care planning, including identifying and addressing individual needs and preferences.
  • Collaborate with the care team to ensure residents receive comprehensive and coordinated care.
  • Document resident care and progress, maintaining accurate and up-to-date records.
Requirements
  • Applicable credentials as required by state
  • A minimum of one year relevant experience working with older adults (preferred)
  • Passion for delivering exceptional care and support to seniors
  • Strong communication and interpersonal skills
  • Ability to work collaboratively as part of a team
