About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Hospice and Home Health team. As a Registered Nurse, you will provide high-quality patient care and support to individuals in their homes and in our hospice facilities.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and other treatments as needed.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Graduation from an accredited nursing program.
  • Current RN licensure in the state of Montana.
  • 1 year of experience in hospice and home health care.
  • National Institutes of Health Stroke Scale Certificate - NIH Stroke Scale Training Course within 90 days of hire.
