About the Role:

As a Comprehensive Healthcare Provider with Matrix Medical Network, you will be responsible for conducting adult and geriatric assessments in various settings, including patient homes, skilled facilities, and through telehealth visits. Your expertise will enable you to identify potential health risks and provide personalized care plans to improve quality of life.

The Ideal Candidate:

  • Must have current Nurse Practitioner licensure in the state of practice and prescription authority or ability to obtain
  • AANP, ANCC, or AACCN certification
  • Current BLS, ACLS, or CPR certification
  • Strong computer skills and familiarity with electronic medical records
  • Excellent verbal and written communication skills
  • Ability to work independently and maintain flexibility with changing priorities
